Before your visit to Dr. Kavali’s office:
We are proud to be a paperless office, with all electronic records. Please visit the Patient Login page on this site to enter and update your demographic and medical history information. You will be given a login name and password when you call to set up your appointment time.

Important Information for our Breast Implant Patients:
If you’re seeing Dr. Kavali regarding any type of surgery involving silicone gel breast implants, please download the following important FDA-required information. Choose the information the suits you–for augmentation (cosmetic) or for reconstruction. This booklet must be reviewed by you at least 1 week prior to your surgery date. Be sure to bring any questions with you to your consultation, so we can be sure to address them with you.
Insurance:
Please call the office at 404-250-3333 to see if Dr. Kavali participates with your particular plan. Note that Dr. Kavali accepts all PPO and POS plans. If you have out-of-network benefits, we will be able to take care of you.

We are pleased to offer you the convenience of using your MasterCard, Visa or American Express card. We also offer CareCredit, a flexible payment option.
